What is the difference with fisheye and wide angle?
I've been looking to get one for my pentax kx. I have also seen them where it says wide angle with macro lens. Do you have to use the macro with the wide angle?
What you are seeing are add-on filters that are neither fisheye nor macro lenses. They are cheap fakes.
A true fisheye lens covers 180 degrees of view and has strong curvilinear distortion.
A Super Wide Angle lens covers just about 110 degrees without any distortion if the focal plane is parallel to the vertical lines in the shot.

A true fisheye lens can cost over $1,000
A good super wide angle lens, costs about $1,900.
You mentioned the term "macro"
